The Staff Memo taped to the window of the Nurses' Station states the following:
The code has been reset to what we agreed on during yesterday's meeting in the doctor's lounge.
Like I said, I don't want anyone writing it down on little pieces of paper for me to find. Be responsible, people.
So, the code shouldn't be written somewhere in the hospital, and it isn't. However, you will find the needed clue for the code when you reach the Doctors' Lounge on the first floor and find the Keypad Combination Note on the bulletin board.

This area becomes accessible after you find the 1F Inner Ward Key in Exam Room 5 on the third floor.
Note: You can skip visiting the Doctors' Lounge and collecting the memo, as the keypad code will always be the same. Simply enter the code for the corresponding riddle difficulty.
Light/Standard
The Keypad Combination Note provides a visual reference for the code. On light and standard difficulties, it looks like this:
The lines are drawn over a nine-circle grid representing the keypad buttons. The four buttons that are crossed are 3, 5, 7 and 8. The code is a simple sequence of these numbers starting from the top – 3578.
Hard
On hard difficulty, the Keypad Combination Note shows a game of tic-tac-toe:
While looking more complicated, the logic is the same. Follow the line drawn over the Xs to get the code – 7456.
