On this page you will find the complete list of Silent Hill 2 Remake (2024) monsters with their descriptions and creator's explanations by Masahiro Ito (beware of some spoilers).

Lying Figure

A horrifying, humanoid creature encased in a tight, fleshy cocoon, resembling a distorted, suffocating straightjacket made of its own skin. Its head is twisted downward and obscured, with only subtle, unnatural movements hinting at the face beneath. Crawling or stumbling forward, it releases acidic sprays from vents in its chest, attacking with a desperate, frenzied motion.

Creator's Commentary:

The design of the upper body of Lying Figure comes from a body bag as a sign of dead body. "Lying Figure" means lying Mary on the bed, also a dead body (Mary's dead body) at the same time. And the animations mean a patient in pain. -Masahiro Ito

The Otherworld version of a Lying Figure has multiple spikes across its body and will spew a cloud of poisonous liquid upon its death, so one must be careful when engaged in close combat.

Creator's Commentary:

In SH2R, the creatures appearing in Otherworld must have surgical tapes on their body surfaces. -Masahiro Ito

Creeper

A small, skittering insect-like creature that scuttles across the ground with unnerving speed. Its grotesque, misshapen body resembles a hybrid of a cockroach and a spider, with a hardened, segmented shell and twitching, spindly legs.

Mannequin

A creature composed of two pairs of feminine legs fused at the waist covered with bandages, creating an eerie, humanoid form that lacks a head or upper body. Its pale, smooth skin and unnatural posture give it a disturbing doll-like quality, while its unsettling movements shift between twitching and lurching with an almost mechanical precision. Mannequins stand still in the dark without triggering the radio and react to James' flashlight attacking him with their upper legs.

Creator's Commentary:

They still refer to the problem that the design is remarkably similar to the work of a great artist Hans Bellmer. But it was just a coincidence. If I had known his work, I wouldn't have done that. It was inspired by a Japanese traditional character, the "ashinaga-tenaga". -Masahiro Ito

Later in game you will come across a faster variant of the Mannequinn that can crawl along walls and ceilings. These will jump at you from above.

Bubble Head Nurse

A nightmarish figure embodying twisted sexuality and repressed desire, a haunting representation of the protagonist's psychological distress. Dressed in a blood-stained nurse’s uniform, the creature’s head is grotesquely swollen and misshapen, with veiny skin stretching over a distorted, blank face, giving it the appearance of a bloated, featureless "bubble." It jerks and twitches as it lurches forward, wielding a rusty metal pipe or a knife with unsettling aggression.

Creator's Commentary:

I made her wear the stockings w/ the color because I was pointed out that the original nurse in the orig SH2 exposed her skin too much and I wanted to mean "this is a remake". Also coz of a game design. -Masahiro Ito

Mandarin

An insect-like creature that lurks just below grated floors or ceilings, with long, grotesque arms reaching upward to grasp at anything above. Its emaciated, elongated body hangs suspended in midair, partially hidden beneath the floor, and its head appears trapped in a fleshy cage-like structure, giving it a tortured, restrained appearance.

Creator's Commentary:

The mouth of Flesh Lip & Mandarin's face symbolize the mouth of Mary verbally abusing James. They were inspired by the mouth of the figure in the center painting of “Second Version of Triptych 1944” by Francis Bacon. As a side note, Mandarin appears only in Otherworld in the SH2R -Masahiro Ito

BTW, in SH2R, the creatures appearing in Otherworld must have surgical tapes on their body surfaces. Also the good-condition hotel is in James' memory of the couple's stay. It means he is still in Otherworld. -Masahiro Ito

Bosses

Pyramid Head

Pyramid Head is a towering, nightmarish figure clad in a blood-stained executioner’s outfit, defined by the massive, rusted metal pyramid helmet that completely obscures his face. He wields a heavy, jagged blade or spear, dragging it ominously across the floor with a chilling metallic scrape that signals his approach. As a relentless, haunting presence, Pyramid Head symbolizes guilt and punishment, embodying the protagonist's darkest emotions.

Creator's Commentary:

I wanted to depict that Pyramid Head is trying to remove the visions from James' mind. Those mean L-Figure, BHNurse, etc. PH trys to make him face to the fact by that. This means his conflict in his mind. He still afraid to face to it at that time so he shoots a gun in the closet. -Masahiro Ito

Flesh Lip

A floating creature suspended from a twisted metal frame, its emaciated body hanging limply as though bound by invisible chains. Pale, veined skin stretches over a featureless face, while its swollen, discolored lips twitch unsettlingly, giving it a disturbing, almost suffocated look. Symbolizing themes of repression and suffering, the Flesh Lip embodies haunting psychological torment as it swings forward in erratic, unnatural motions to attack.

The cages of Flesh Lip & Boss Mary symbolize Mary on a sick bed. Also the cage is similar to the ones in the painting coz James saw it when he visited the town w/ Mary. But I made both of them upside-down to negate they were victims. Upside-down Maria,too. -Masahiro Ito

Abstract Daddy

A fleshy mass bound to a stained, bed-like frame, with distorted human forms writhing against each other as if trapped in perpetual torment. Its fused, faceless bodies and twisted limbs suggest themes of abuse and repression, moving toward the player with unsettling jerks and spasms. Representing deeply buried trauma and pain, the Abstract Daddy is a haunting manifestation of suppressed memories and unresolved suffering of Angela.

Creator's Commentary:

The reddish surface of the SH2R abstract daddy symbolises something. Because that creature is different from other creatures in SH2R. -Masahiro Ito

All creatures in Silent Hill 2 except Pyramid Head are female since they are James' projections of Mary. As an exception, Abstract Daddy is a creature being projected by Angela, which James and we are looking at by his perspective. -Masahiro Ito

Mary

A twisted, horrifying version of Mary, reflecting James's conflicted emotions and guilt. Her form becomes an unsettling hybrid, merging human features with grotesque distortions, and her movements are erratic, shifting between pleading and attacking with unnatural speed.
